Please see the Player Assessment Worksheet and Player Rating PDF files attached below to assist you in completing Player Assessments for each of the players listed on your roster. Player Assessments are crucial to our region's ability to form "Balanced Teams" fulfilling one of the 6 Philosophies in AYSO.

Rating Guidelines: Rate each Player using a rating of 1 through 5 as listed in the attachments. 
Base your ratings according to how each Player performs while being evaluated (can be on the Field during a Match, during practice or at a Player Assessment)
